We register patients in the order of registration. You will be called as soon as we can finalize your registration.
If your postal code falls between 6842AA and 6846ZZ, you can register with our practice.
We request that you hand in the registration form at the practice. When you come to hand in the forms, the assistant will ask for a valid form of identification (passport, driver's license, identity card).
Each person needs their own registration form; one form is not enough to register an entire family at once. Every person over the age of 12 must come in person to the reception desk with proof of identity.
You will find a brochure with the registration form providing information about exchanging your medical records with the GP out-of-hours service and other healthcare providers.
If you have recently moved to Arnhem, it is also advisable to register with a pharmacy. Pharmacies in our neighborhood are Pharmacy Elderveld and Pharmacy De Laar.
