Registering as a new patient:
- Welcome: Have you recently moved or are you looking for a (new) permanent general practitioner (GP) in the area? You are very welcome to join us.
To ensure a smooth registration process, we kindly ask you to keep the following in mind:
- Registration form: You can fill out the registration form via our website or pick one up from the assistant at the reception desk. Please fill out a separate form for each family member.
- Identification & Insurance card: Upon final registration, we are legally required to check your proof of identity (passport, ID card, or driver's license) and your health insurance card.
- Medical records: Do not forget to inform your previous GP about your switch, so they can securely transfer your medical records to us digitally.
Are you temporarily in the area (for example, for a holiday, work, or visiting family) and in need of urgent medical care? We are happy to help you.
Care for temporary visitors (passers-by):
- Always call first: In case of acute symptoms, please always contact us by phone first to schedule an appointment. Do not come to the practice without an appointment.
- What to bring: Please bring a valid form of identification (ID card, passport, or driver's license) and the details of your own general practitioner (GP)
- .Costs (Temporary visitor rate): Consultations are charged according to the statutory rate for temporary visitors (passantentarief). Depending on your situation, you can pay directly by debit card at the reception desk, or we can submit the claim to your health insurance company.
You will receive a receipt or invoice that you can submit to your own health insurance provider.
